Role Description

This is a full-time role as a Registered Nurse Case Manager at Dignity Home Health and Hospice located in Salt Lake City, UT providing care to those in Salt Lake County. The Registered Nurse will be responsible for:

  • Coordinate the overall care of the patient while interacting with patient, family, physicians, Interdisciplinary Team members and other healthcare professionals.
  • Provide professional nursing care for both home health and hospice patients in their place of residence.
  • Complete initial, comprehensive and ongoing assessments for patients to determine their needs.
  • Conduct nursing visits for pain and symptom management, wound care, medication administration and other skilled care as ordered by the physician.
  • Complete all documentation and charting accurately and in a timely manner.
  • Monitor and supervise those involved ensuring that the patient receives the best possible care.


Qualifications

  • Strong clinical skills and knowledge in nursing
  • Experience in providing direct patient care
  • Ability to assess patient conditions and develop care plans
  • Proficiency in administering medications and treatments
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively with healthcare team members
  • Attention to detail and organizational skills
  • Current RN license in the state of Utah
  • Bachelor's degree in Nursing or related field
  • Experience in home health or hospice care is a plus
