Summary

As a member of the PACE Interdisciplinary Team, the Enrollment Registered Nurse participates in the assessment and enrollment/intake process of the PACE program. The nurse works with the care team to ensure participants in the PACE program are qualified and census of the program is within the mandated range. New grads and experienced nurses encouraged to apply Hours are 8a-4p, Monday through Friday.

This is an in person position, with frequent local travel.


Responsibilities

  • Conduct initial appointments to explain the PACE model to potential participants and their caregivers
  • Ensure potential participants meet eligibility criteria in accordance with PACE enrollment rules
  • Use a strength-based approach to evaluate the current needs and health status of potential participants
  • Communicate with the care team regarding the current status of prospective participants
  • Mediate and prioritize between prospective participants'/caregivers' needs and the care team's concerns
  • Participate in weekly meetings to provide clinical insights, advice, and support to the care team
  • Gather and maintain accurate data on new referrals in the CRM, EMR software, and MassHealth Virtual Gateway
  • Visit prospective participants in their homes to complete and submit the state-required Minimum Data Set-Home Care (MDS-HC)
  • Conduct comprehensive needs and medical assessments, including evaluations of current functional status, caregiver status, current services, and home safety
  • Present and discuss initial needs assessment intake with the PACE IDT
  • Determine the need for and facilitate additional assessments for prospective participants
  • Collaborate with the IDT to assess the ability of prospective participants to safely remain in the community with PACE services
  • Communicate enrollment status and progress with prospective participants and other involved members

Requirements:

  • Current RN license in Massachusetts, or ability to obtain before start date
  • Current CPR Certification
  • Frequent local travel with access to personal vehicle
  • Covid Vaccination and fit test required
