Responsibilities:

As a Registered Nurse (RN) at Suncrest Hospice, you will be responsible for providing compassionate care and support to our patients, conducting visits at various locations such as private homes, Skilled Nursing Facilities, and assisted living facilities.

You will collaborate with an interdisciplinary team, utilizing your expertise in psychosocial assessments, interventions, and knowledge of community resources.

Depending on the role, you may be involved in Case Management, Admissions, or Resource allocation.

Qualifications:
  • Active Registered Nurse (RN) license in the state(s) of practice
  • Ability to work effectively in a team setting with other healthcare professionals
  • Experience in one-on-one patient interactions with strong team support
  • Proficient in time management for scheduling visits, documentation, and meetings
  • Knowledge of hospice regulations and excellent communication skills
  • Proficiency in electronic medical records
  • Valid driver's license, reliable transportation, and auto insurance for travel within the service area
