Sterling, United States Rock River Hospice & Home Full time

Purpose: Provides personal care and related services in the home, nursing home or hospital for patients as assigned by the Director of Nursing.

 

Reports To:    Director of Nursing

 

Qualifications: Graduate of a certified nursing assistant program

                       Ability to read, write and speak English fluently

                       Demonstrated clinical ability through written and skill test

                       Ability to lift 60 pounds

                       CPR certified

Responsibilities (including, but not limited to):

  • Aid the patient in maintaining good personal hygiene by assisting with bathing; care of skin, hair, nails, and mouth.
  •  Changing of bed linens.
  •  Assists patients to get in and out of bed and to ambulate.
  •  Assist patients in the use of durable medical equipment.
  •  Report to the DON/primary care nurse any change in the patient’s situation and/or physical or psychosocial condition.
  •  Collect specimens, provide treatments, take vital signs, change dressings, and assist with care of catheters and ostomies as directed by DON.
  •  Assist in feeding patients when necessary and as directed by DON.
  •  Maintain a healthy, safe, and clean environment for the patient.
  •  Perform homemaking duties as assigned.
  •  Assist patients with prescribed exercises, when required.
  •  Maintain up-to-date clinical knowledge by attending at least twelve hours of CNA training per year and other in-service training as requested.
  •  Attend interdisciplinary, staff, and CNA meetings as required.
  •  Maintain documentation as required by RRHH policy, Medicare, and accreditation requirements.
  •  Perform other duties as requested.
  •  Adhere to the practice of confidentiality regarding patients, families, staff, and the organization.

Completes annual TB testing in month due.

3.   Understanding of and commitment to the hospice concept, philosophy, and program.

4.   A desire for continual personal and professional growth and development.

5.   Physical ability to lift sixty (60) pounds.
