Monday - Friday

· Daily production requirements vary depending on census.


Supervisory Responsibilities:

· None.

Duties/Responsibilities:

· Perform evaluation and medical management services for Long Term Acute Care (LTACH) patients.

· Admits patients and completes all related documentation.

· Initiates plan of care for newly admitted patients.

· Perform dictation of H&P examinations.

· Does related follow up for patient issues and cross-coverage for the hospitalist team.

· Collaborate with hospitalist providers in developing and providing medical management.

· Is alert to any changes in condition and initiates early management to sustain stay and cause a return to a higher level of care to be unnecessary.

· Evaluates major change in condition promptly and reports to hospitalist.

· Work closely with the therapy and nursing team to assure strong interdisciplinary plan of care.

· Perform such other responsibilities as reasonably assigned by the chief hospitalist and with the NP/PA scope of practice.

· Participates in call from 8 AM the first day of rotation to midnight on the last day of rotation.

Education and Experience:

· Successful completion of an approved nurse practitioner or physician assistant program.

· Must have a current, unrestricted advanced practice licensee in the state of South Carolina.

· 1-2 years of recent hospital experience.


Physical Requirements:

· Requires the ability to sit at a desk for period of time and intermittently walk, stand, stoop, kneel, crouch, and reach while performing office duties.

· Life and or move light weights

· Use hands to finger, handles or feel objects, tools, or controls.

· Must be able to maintain effective audio, visual discrimination and perception needed for making observations, communicating with others, reading, and writing, and operating office equipment.

· Must be able to use telephone to communicate verbally and a keyboard to communicate through written means, to review information and enter/retrieve data, to see and read characters on computer screen.

· Requires the strength and stamina to perform clinical duties.

· May be exposed to adverse indoor and outdoor environmental conditions, bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards.
