Brief overview of Responsibilities:

Under the direction of an RN, assist with the preparation, implementation and continuing evaluation of the patient care plan.
Recognize and understand the effects of social and economic problems upon patients and provide for the emotional and physical comfort and safety of patients and their families.
Prepare clinical and progress notes based on patient visits.
Performs an ongoing assessment during each visit and documents data
Observe, record, and report to the appropriate person patient symptoms, reactions and changes, as appropriate.
Assist the Registered Nurse in performing specialized procedures as directed by the Registered Nurse under physician's orders.
Participates in on-call duties as defined by on-call policy
Other duties as assigned

Qualifications:

Must be a Licensed Practical Nurse with a current license to practice nursing in the Commonwealth of Virginia
At least one year of hospice experience required.
Graduate of an accredited nursing school.
Must have a valid driver's license and a reliable car available for use during work hours.
