Therapist for Occupational Rehabilitation

The Occupational Therapist plays a vital role in delivering restorative and rehabilitative therapy services. You will collaborate closely with the PACE Physician, rehabilitation staff, and other IDT members to promote participant independence, safety, and improved performance in Activities of Daily Living (ADLs). Build lasting relationships with participants for enhanced care over time rather than short-term rehab schedules. Collaborate effectively with a diverse team of therapists and healthcare providers. In your day-to-day role, you will: Conduct thorough Occupational Therapy assessments within our facility and occasionally in participant homes to identify care needs, preferences, and goals. Actively contribute to IDT meetings to collaboratively develop participant care plans. Deliver and document therapeutic interventions as outlined in care plans. Administer active and passive exercises, muscle re-education, gait training, and functional training for ADLs. Supervise and instruct Therapy Assistants to effectively implement Functional Maintenance Programs (FMP). Job requirements include: Bachelor's degree in Occupational Therapy; a master's degree is preferred. A minimum of two years of clinical experience working with a frail or elderly population is preferred. A valid Occupational Therapy License in California or proof of eligibility for licensure. Current CPR certification is preferred. Ideal candidates will have experience in outpatient, acute, inpatient, or skilled nursing settings.
