The hospice case manager also helps families navigate during this difficult time by providing care and support.

Hospice Case Manager Job Duties

Hospice case managers have a wide range of responsibilities including

  • Meeting regularly with patients and their families to monitor the patient's physical, emotional, and spiritual needs
  • Assessing patient's social and medical history to develop plans of care that meet their needs
  • Working with physicians, nurses, and other medical professionals to coordinate care and treatment plans for each patient
  • Maintaining records of patient care plans, physician orders, medication administration, and other information about a patient’s condition
  • Coordinating care with other medical providers and social services agencies in order to meet patient needs
