Job Summary
We are seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality care to our patients in their homes. As a Home Care LPN, you will work under the supervision of a Registered Nurse and be responsible for providing direct nursing care to acutely ill and chronically ill patients. Your duties will include recording clinical and progress notes, coordinating patient care activities, and working collaboratively with other healthcare team members to provide the highest quality of patient care.

Responsibilities
• Provides high-quality clinical services within the scope of practice, as defined by state laws governing the practice of nursing, in accordance with the plan of care and in coordination with other members of the healthcare team.
• Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record.
• Actively participates in the agency's care coordination process, including timely reporting patients' needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, and other clinicians.
• Ensures confidentiality of patient health information per agency policy.

Requirements
• Must meet educational/experience requirements outlined in the laws of licensure states.
• A minimum of one year of acute care experience is preferred, as well as previous home care experience.
• Must have current LPN licensure in the state of practice and a current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
