Speech Therapist Opportunity at Atrio Home Health - Grand Rapids:


Atrio Home Health in Grand Rapids is currently looking to expand its team by adding a per diem Speech Language Pathologist.

In this role, you will have the opportunity to work with clients in the comfort of their homes, evaluating and treating speech, language, swallowing, and cognitive disorders.


Responsibilities:

  • Conduct speech therapy evaluations to monitor client progress and suggest care plan adjustments
  • Diagnose and interpret speech, language, swallowing, voice, and cognitive disorders through various assessments
  • Complete OASIS discharges and recertification paperwork following guidelines
  • Collaborate with physicians and care team to develop individualized treatment plans
  • Coordinate services in communication with other Atrio Home Health staff
  • Offer speech therapy treatment to designated clients
  • Identify obstacles to desired outcomes and assess care effectiveness
  • Educate clients and their families on communication, cognitive, and swallowing skills
  • Participate in interdisciplinary team meetings and maintain communication for seamless client care
  • Prepare essential clinical documentation and progress notes

Qualifications:

  • Active Speech Language Pathologist License in Michigan
  • Possession of a valid Michigan driver's license, auto insurance, and reliable transportation
  • Preferred 2 years of experience, especially with the elderly
  • Dedication to providing topquality care to older adults
