Gouverneur provides primary and specialty outpatient care through its Diagnostic and Treatment Center (D&TC) and residential care for the elderly, who are infirmed in its 295-bed Skilled Nursing Facility (SNF).NYC Health + Hospitals/Gouverneur Skilled Nursing Facility provides 24-hour care for individuals needing short-term, long-term care, or hospice care. US News and World Report rated The facility as the Best Nursing Home. Post-Acute Care services include physical therapy, occupational therapy, speech and audiology, and vocational rehabilitation.


POSITION OVERVIEW:

Under supervision, provides professional nursing care to residents within an assigned unit. Diagnoses and treats responses to actual or potential health problems using nursing methods and techniques such as case-finding, health teaching, health counseling, and provision of care supportive to and restorative of life and well-being.


• Interacts with all persons (including residents, family members/visitors, workforce members, volunteers, vendors, and others) based on the organization’s ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence).

• Develops, implements, and continuously evaluates the nursing regimen for assigned residents.

• Adheres to the nursing process when providing care/services to residents (assessment, nursing diagnosis, planning, implementation, and evaluation).

• Formulates and implements comprehensive, person-centered nursing care plan. Sets priorities, delivers, and directs care to meet resident needs.

• Records and maintains nursing care plans and progress notes on residents to ensure continuity of care.

• Ensures that medical and nursing orders are accurate, implemented, and evaluated based on facility policy/procedures.

• Document care and services rendered are recorded in the medical record promptly and accurately.

• Continuously evaluate residents’ responses and make appropriate changes to the care plan. Maintain accurate documentation as described in policies and procedures.

• Interprets and reports residents' responses to supervisor and/or health team members as necessary.

• Collaborates with other interdisciplinary team members in the care of residents, including the resident, family, and/or significant others. Discusses education and discharge planning issues as they arise.

• Fosters a safe and caring environment and promotes the well-being of residents, including abuse/neglect prevention, person-centered care planning, and delivery, and serves as an advocate for residents' and families' needs.

• Determines and assigns those components of the nursing regimen that may be carried out by allied nursing personnel.

• Provides teaching, guidance, and direction to allied nursing personnel in rendering resident care. Ensures allied nursing personnel and licensed practical nursing staff provide care based on the resident’s care plan and document it in a timely/accurate manner.

• Communicate effectively and compassionately with residents and their loved ones/healthcare representative(s), including resident changes in condition and other updates.

• Communicates with workforce members, including supervisors, through team meetings/huddles to disseminate information routinely.

• Maintains professional competence through participation in continuing education and other appropriate learning experiences. Completes all mandatory training/in-services that are assigned promptly.

• Participates in the mandatory Nurse Residency Program (NRP) during the first year of employment to improve critical thinking skills and evidence-based practice.

• Performs other professional nursing activities as delegated by the nursing service administration.


MINIMUM QUALIFICATIONS


For individuals holding an unrestricted registered professional nursing license BEFORE July 1, 2020, in New York State, another state, or United States territory:

1. A valid New York State license and current registration to practice as a Registered Professional Nurse

in New York State; and

2. Holds or obtains a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA) through facility orientation.


For individuals issued a registered professional nursing license on or AFTER July 1, 2020,

based on the expected implementation of Chapter 502 of the Laws of 2017 and Chapter 380 of the

Laws of 2018:

1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and


a. A Bachelor’s of Science degree in Nursing or higher (or other nursing degree, as identified by NYS Education Department, including Master of Science in Nursing, Doctor of Nursing Practice, Doctor of Philosophy Degree in Nursing or Doctor of Nursing Science) from a accredited college or university, registered with the NYSED; or


b. Obtains a Bachelor of Science Degree in Nursing or higher, as described in “a” above, within ten (10) years of the date of initial licensure; and

2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support

(BLS) through the American Heart Association (AHA).


Special Note:

For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and application for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of an NYS-licensed, currently registered RN for up to 90 days immediately following graduation, pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a restricted permit is received, individuals can practice nursing under the immediate and personal supervision of an NYS licensed, currently registered RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, one must hold or obtain, through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
