Job Summary:

The primary function of this role is to process all aspects of scheduling nursing visits via our Home Care Home Base software system.

Key Responsibilities:
  • Schedule and document patient schedules in a timely manner.
  • Coordinate care with management and patient interdisciplinary teams.
  • Provide effective communication to patients, staff members, and other healthcare professionals.
  • Respond to agency and patient needs in a professional and creative manner.
  • Demonstrate commitment, professional growth, and competency.
  • Promote Agency philosophy and administrative policies to ensure quality of care.
Requirements:
  • Minimum 1 year of home health experience.
  • Excellent computer skills required.
  • Excellent interpersonal and organizational skills.
  • Basic knowledge of scanner/fax/copier equipment.
  • Phone etiquette skills required.
  • Ability to take direction with a multitude of tasks.
  • Typing ability 40-50 WPM.
  • Knowledge of Medicare regulatory guidelines.
Qualifications:
  • Graduate of an accredited school of practical/vocational nursing.
  • Current state license as an LVN/LPN.
  • Current State's Driver's License.
