Home Care RN Case Manager

RNs will have the opportunity to collaborate with multiple OT/COTAs, PT/PTAs, SLPs, and LPNs within the company to problem-solve and provide optimal care to patients.
We have a $1500.00 sign-on bonus of which $500.00 payable after 60 days of full-time employment, $500.00 payable after 90 days and $500.00 payable at 120 days of full-time employment. Nurses will schedule their own patients with the expectation that OASIS admissions and resumption of care. All OASIS are completed within 48 hours of referrals and documentation has been completed within 48 hours of each visit.
Oregon Continental offers 40 hours of front-loaded PTO every January 1st along with 16 hours of banked sick time for our full-time hourly employees, that is prorated to your hire date. This PTO can be utilized after 90 days of employment, 1-week PTO vacation after 1year of full-time employment on your anniversary date. Oregon Continental promotes good work life balance and allows time off during the 90-day probationary period and when PTO is not available with a 2-week notice.
OCHHC offers Medical, dental, vision and life insurance within 30-60 days of employment depending on your hire date. Medical benefits are paid at 50% for full time hourly employees only.
