The ideal candidate will demonstrate a strong commitment to delivering exceptional care to patients in their homes.
POSITION SUMMARY
The Registered Nurse (RN) will be proficient in planning, organizing, and overseeing home care services with a focus on community health education. The professional RN will utilize available resources to plan and direct services tailored to meet the needs of individuals and their families within their homes and communities.
  • This is a Monday to Friday position; PRN may be considered for the right candidate.
  • Conducts initial assessments of patients and families to identify home care needs, including comprehensive physical assessments and medical histories.
  • Regularly reassesses patient nursing requirements.
  • Initiates and updates the care plan as patient conditions and needs evolve.
  • Utilizes health assessment data to establish nursing diagnoses.
  • Develops a care plan in collaboration with the patient and family, setting goals based on nursing diagnoses and incorporating therapeutic, preventive, and rehabilitative nursing actions.
  • Implements appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct patient care as outlined in the State Nurse Practice Act.
  • Offers counseling to patients and families to address nursing and related needs.
  • Delivers health care instructions to patients as appropriate based on assessments and care plans.
  • Identifies discharge planning needs as part of care plan development and implements them prior to patient discharge.
  • Acts as Case Manager, coordinating patient care for an assigned caseload.

QUALIFICATIONS:
  • Associate's or Bachelor's degree with at least one year of home health care experience preferred.
  • Must hold a valid Registered Nurse license in the State of Arizona.
  • Current BLS certification required.
  • Licensed driver with a reliable and insured vehicle.
  • Strong observational, verbal, and written communication skills, along with problem-solving abilities and basic math skills as per competency checklist.
  • Visual acuity and hearing ability to perform required tasks.
  • Familiarity with EMR systems (preferably Devero).
