Job DescriptionSchool Social Worker - Wakefield, Rhode Island Connected Health Care Pay Rate: $50/hr Schedule: Full-Time, Onsite Grades: 6th - 8th Connected Health Care is seeking a dedicated and compassionate School Social Worker to join one of our trusted school partners in Wakefield, Rhode Island. This is a full-time, onsite position working with middle school students in grades 6-8. The ideal candidate will be currently certified by the Rhode Island Department of Education (RIDE) as a School Social Worker and have a strong background in providing emotional, behavioral, and academic support to students within a school setting. Key Responsibilities: Provide individual and group counseling to students to address academic, social, and emotional challenges. Serve as a key member of multidisciplinary teams, including the Evaluation Team, IEP Team, Crisis Intervention Team, and RTI/MTSS Team. Collaborate with teachers, administrators, and families to design and implement interventions that promote student success. Support the development and implementation of behavioral intervention plans. Participate in crisis response and provide social-emotional support to students and staff as needed. Maintain accurate documentation and ensure compliance with district and state guidelines. Qualifications: Current RIDE certification as a School Social Worker (required). Master's Degree in Social Work (MSW) from an accredited institution. Prior experience in a school-based setting preferred. Strong communication, collaboration, and problem-solving skills.
