\*\*Overview:\*\*

Join our healthcare team as a Hospice Nurse dedicated to providing compassionate end-of-life care. We are looking for a caring and skilled individual to deliver quality patient care in a hospice setting. As a Hospice Nurse, you will work collaboratively with a multidisciplinary team to ensure the comfort and well-being of patients during their final journey.

\*\*Responsibilities:\*\*

- Conduct patient assessments and develop personalized care plans tailored to individual needs

- Administer medications, treatments, and IV infusions as prescribed by healthcare providers

- Monitor patient vital signs, symptoms, and pain levels, promptly reporting any changes

- Perform specimen collection processing and assist in sterile processing procedures

- Provide infectious disease care and maintain aseptic techniques to prevent infections

- Offer urgent care interventions when necessary and manage medical equipment (DME)

- Deliver ostomy care, memory care, and conduct physical examinations as required

- Collaborate with physicians, therapists, and other team members to ensure holistic patient care

\*\*Qualifications:\*\*

- Active RN license in the state of practice

- Minimum of 2 years of experience in a clinical setting or hospice care

- Proficiency in ICD-9 coding, patient assessment, and care planning

- Strong knowledge of medical terminology, procedures, and sterile processing techniques

- Excellent communication skills and ability to interact compassionately with patients and families

\*\*Nice-to-Have Skills:\*\*

- Experience with IV infusion therapy procedures

- Familiarity with eClinicalWorks or similar EMR systems

- Proficiency in venipuncture for blood draws and IV therapy

- Prior exposure to memory care practices

*REQUIRED CREDENTIALS:*
* RN License (GA) (SC) -We are a multi-state office
* CPR certificate
* TB skin test or x-ray
* 2 valid forms of identification
* COVID vaccine preferred, but not required
* 1 year experience in CNA/ Caregiving
