Affinity Health Center is looking for an experienced Advanced Practice Provider (APP), Family Nurse Practitioner or Physician Assistant, to deliver compassionate and evidence-based health care and to partner with our patients in providing full primary care services including preventative/wellness, chronic disease management and acute-episodic care. The APP is a member of the patient care team working with their collaborative physician and medical leadership team to provide care for walk-in patients at Affinity. The APP Midlevel Provider will be responsible for compliance with government regulations related to the provision of healthcare. TheAPPparticipates in the Continuous Quality Improvement activities of the organization and the Performance Management System.
