The Trent Hill Center Group Home Care Coordinator is responsible for evaluating and assessing the provision of all essential tasks for children who are receiving care in Trent Hill Center's group foster care home. The Care Coordinator shall be available for supervision and discussion during program hours to ensure that children are receiving care in a safe and efficient manner. Those hours must typically occur at a time the children are expected to be awake and on-site at the program. 


The Care Coordinator duties must be rendered by a person holding a bachelor’s degree from an accredited college or university in a human services field; a bachelor’s degree in a non-human services field and having a minimum of one (1) year of experience working with the population to be served; or an associate degree from an accredited college or university, in a human services field, and having a minimum of three (3) year experience working with the population served. The Human Services field may include, but is not limited to, the following: psychology, social work, early childhood education, child development, or a related field, such as criminal justice, rehabilitative counseling, or elementary or secondary education.


Care Coordinator responsibilities include, but are not limited to: 

  • At least one (1) hour per week involvement in child/youth specific responsibilities dealing with crisis management, daily living skills, peer relationships, future planning and/or discharge.
  • At least one (1) additional hour per week in other related duties including intake discussions, staffing of cases, evaluations of workers, teaching, and administrative duties.
  • Must spend a portion of their time watching and interacting with each child/youth.
  • Must meet weekly with direct care staff either individually or in groups, to discuss specific children’s cases to monitor the child’s behavioral, social, emotional, educational, vocational, nutritional, spiritual, cultural and permanency needs. This meeting will be documented in the child’s summary notes. Summary notes will be uploaded into the CAIP.
