Job Overview:
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our team on a per diem basis, specifically focusing on continuous care. The LVN will play a crucial role in providing high-quality, personalized care to patients in their homes during challenging moments in their end-of-life journey. The ideal candidate will possess excellent clinical skills, a deep understanding of hospice care principles, and the ability to provide continuous care to patients and their families.
Responsibilities:
Continuous Care Provision:
· Deliver continuous nursing care to patients requiring more intensive support during critical periods.
· Administer medications, treatments, and interventions as prescribed by the hospice physician.
· Collaborate with the interdisciplinary team to develop and adjust individualized care plans.
Patient and Family Education:
· Educate patients and their families on the goals of hospice care and help them make informed decisions about treatment options.
· Provide guidance on symptom management and end-of-life care processes to enhance the overall understanding of the hospice journey.
Documentation:
· Maintain accurate and up-to-date patient records, documenting all care provided, assessments, and interventions in accordance with hospice policies and regulatory guidelines.
Compliance and Quality Assurance:
· Adhere to all relevant regulations, standards, and hospice policies to ensure the delivery of safe and effective care.
· Participate in quality improvement initiatives to enhance the overall quality of care provided.
Qualifications:
· Current LVN license in the state of practice.
Minimum of 2 years of experience in nursing, with a preference for hospice or palliative care experience.
· CPR certification and other relevant certifications as required by state regulations.
· Excellent communication and interpersonal skills.
· Ability to work independently and collaboratively within a team.
· Compassionate and empathetic approach to end-of-life care.
Schedule:
Per diem, flexible schedule based on the needs of patients.
