Assertive Community Treatment (ACT) Psychiatric Rehabilitation SpecialistACT Psychiatric Rehabilitation Specialist Position Summary: This position is part of an interdisciplinary team that provides quality evidenced based practices to clients living in the community struggling with behavioral health issues. This is a full-time position. Job Responsibilities: Initiate member assessments, develop person centered individual recovery plans, and document progress towards achieving goals, other related documentation requirements. Positive role modeling for members and peers. Provide psychiatric rehabilitation using evidenced based practices and facilitate groups once per week. Provide quality services to fulfill the ACT mission of providing sanctuary and support to members in the community. Assist in coordination of services provided to our members from other community agencies. Qualifications, Education and Experience:Associate's degree in the Human Service field and/or two years of professional experience working with chronically mentally ill clients. Certified Rehabilitation Counselor (CRC) certification preferred. Current CPR and First Aid certification Valid full driver's license with a safe driving record. Excellent verbal and written communication skills. One year's experience in teaching and/or social work is preferred. Why work for HealthPort?
