*Responsibilities:*
* *Direct Patient Care:*
* Administer medications and treatments as prescribed by physicians.
* Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, eating, and toileting.
* Monitor and record vital signs including blood pressure, pulse, temperature, and respiration.
* Perform routine nursing procedures such as wound care, catheter care, and colostomy care.
* *Assessment and Documentation:*
* Conduct regular assessments of residents' physical and mental health.
* Document residents’ conditions, care provided, and responses to treatments in medical records.
* Report significant changes in residents' conditions to the supervising RN or physician promptly.
* *Coordination of Care:*
* Collaborate with other healthcare professionals to plan and deliver comprehensive care.
* Coordinate with family members and caregivers regarding residents’ care plans and progress.
* Participate in interdisciplinary team meetings to discuss residents’ care plans and outcomes.
* *Health Education:*
* Educate residents and their families about health conditions, treatments, and wellness practices.
* Promote healthy lifestyle choices and preventive health measures among residents.
* *Safety and Compliance:*
* Adhere to all facility policies, procedures, and protocols.
* Follow infection control practices to prevent the spread of diseases.
* Ensure a safe and clean environment for residents, staff, and visitors.
* Comply with state and federal regulations governing long-term care facilities.
* *Emergency Response:*
* Respond promptly to medical emergencies and initiate appropriate interventions.
* Perform CPR and other emergency procedures as necessary.
* Assist in the development and implementation of emergency preparedness plans.
