Minimum Qualifications:

  • Bachelor's Degree or higher + 1 year of experience with quality improvement work in a healthcare or related setting.
  • Training in quality improvement methods such as Model for Improvement or LEAN and experience applying in work setting.
  • Strong data skills required, including the ability to create and interpret performance dashboards across multiple measures.
  • Strong project management and public speaking skills.
  • Exceptional people skills - able to engage and motivate individuals from a variety of backgrounds and disciplines.
  • Able to work independently and self-direct, as well as work effectively as part of a team.
  • Strong desire to help others.


Responsibilities:

Practice Facilitator will join an already existing team of quality improvement facilitators working with small primary care practices in Los Angeles to implement quality improvement performance reporting, QI teams and population management.


Facilitators will:

(a) Create a work plan with individual practices for meeting specified performance benchmarks.

(b) Provide technical assistance to small primary care practices to assist them in attaining identified benchmarks including: practice recruitment and retention in transformation work, assessment of practice readiness to engage in improvement work,  workflow redesign, elbow support, resource identification, HIT optimization, project management, enhanced population health practices, improved care delivery, data collection, analysis and performance reporting and strategic planning among others.

(c) Participate in weekly collaborative sessions with other facilitators/coaches for training and to share exemplar practices and problem solve issues at specific practices.

(d) Gather performance data from practices and analyze data and prepare performance reports for practice and for LA Net clients. 

(e) Document work on LA Net PF platform. 

(f) Assist in PF program development and improvement.


Position type and details

This is a 1099 position. Individuals hired will be responsible for their own professional liability insurance, travel, and costs associated with delivering services to LA Net clients.

The consultant/contractor will be expected to provide all equipment, materials, etc. required to carry out quality improvement tasks and deliverables for LA Net.

Location of work is throughout LA County. Some virtual work is also likely.

The consultant/contractor will work closely with the LA Net client and their team to provide support and technical assistance.


Compensation, hours and location

Fixed cost contract: $80,000 for up to 1800 hours of TA support over a 12 month contract period. Location of work is LA County.
