YOUR ROLE AS A LICENSED PRACTICAL NURSE

Knowledge and skills required for the position are:
LPN provides initial and ongoing assessments to identify the client's medical needs
level of functioning and ability to communicate and learn.
LPN initiates the plan of care
necessary revisions
and performs OASIS assessments at appropriate time points when applicable.
LPN Initiates and performs appropriate nursing interventions.
LPN coordinates services
informs the physician and other personnel of changes in the client's condition and needs.
LPN collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
LPN Prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.
