Description

NorthPoint Health & Wellness Center, a state-of-the-art multicultural, community-based medical and dental clinic is seeking a Care Coordinator to join their team. The Care Coordinator will work with primary care providers, clinic staff and other collaborative teams to assist in the identification of basic needs, organize activities, and collaborate with other health care team members to coordinate and implement plans of care for patients. Activities will include services for women that are pregnant and/or have chronic conditions, in the effort of linking them with health and community services/partners that will promote self-management, improve health outcomes and reduce disparity. Current Hennepin County employees who refer a candidate hired into an open competitive position may be eligible for a  $500 r eferral bonus. Location and hours:
This position is located at the NorthPoint Health & Wellness Center at 2220 Plymouth Avenue North, Minneapolis, MN 55411. Work hours are typically Monday through Friday from 8:30 a.m. - 5 p.m.

This is a full-time, benefit earning position. This position is internally classified as Case Management Assistant.

In this position, you will:

Assess, plan, and coordinate patient care according to established clinic policies and procedures, objectives, performance improvement activities and safety, as well as environment and infection control standards. Manage a caseload of OB patients: follow patients closely for prenatal visits, community resources and additional needs.  Facilitate collaboration between community health educational resources and the patients to meet the patients’ health care needs. Identify clinic and community resources and coordinate appropriate referrals. Document and monitor patient care visits in EPIC, the electronic medical record. Maintain accurate records and ensures confidentiality of all data collected during health promotion and prevention services. Need to have:
One of the following:Two years or more of responsible, paid social service experience providing direct client service.A two-year human services certificate.Two years or more of college coursework in the behavioral sciences. Nice to have:
Ability to communicate fluently in Spanish in addition to English, both verbally and in writing. Experience:Using computer programs such as EPIC to perform data entry where accuracy is essential.Providing case management in a clinic setting.Working with multiple providers, staff and collaborative partners to improve the care to diverse and underserved people with a variety of cultural and language barriers. Knowledge of community resources and social services available to clients. Ability to:Work effectively and communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner.
