Program Specialist

Summary

This position serves as the Program Specialist - Mental Health Patient Care Management Module Coordinator (MH PCMMC) for the facility. The Program Specialist works in collaboration with the Chief of Mental Health, Administrative Officer, Clinic Coordinators, Providers, and other clinical staff in assuring the management and accuracy of the provider panel data base.

Duties

Duties and responsibilities include, but are not limited to : Enters Mental Health information into PCMM system, enrolls patients into the Mental Health teams, and assigns patients to the appropriate provider within the PCMM system. Performs work in direct support of an administrative area such as management analysis or operating program analysis for the MH PCMM package. Works with Team Leaders and Clinic Managers to formulate and conduct training and orientation for all staff, providers, and clerks involved with MH PCMM. Provides assistance and technical level support from within the Mental Health Service or a comparable setting. Performs work in support of general management and/or program analysis functions in which knowledge of the purpose, operation, methodology and techniques characteristic of specific management/program analysis functions is required: specifically for the MH PCMM, which includes interdisciplinary panels and maintenance of those panels. Facilitates the completion of required Clinical Pertinence reviews for both Psychology and Prescribing clinical staff.
Assists, or provides training, implementation, monitoring and evaluation of all administrative procedures related to PCMM operations, as well as work closely with the clerks in the Mental Health settings serving as a MH PCMM resource person, and will certify MH PCMM application users to ensure competency. Serves as the MH Patient Care Management Module Coordinator (MH PCMMC) for Mental Health Teams within the Health Care System (HCS) service area. He/she works in collaboration with the team leaders, Clinical Managers, Providers, Nurses, Clerks, and the rest of the clinic staff, CBOC staff and contract providers and is responsible for maintaining the integrity of the PCMM database for all of Mental Health throughout the hospital, outpatient clinics and CBOC's service area. Works with Team Leaders and Clinic Managers to formulate and conduct training and orientation for all staff, providers, and clerks involved with MH PCMM. Provides expertise and advice to all providers involved with MH PCMM. Obtains, organizes, files and retrieves reports and numerical data for use in preparing status reports, briefings and presentations. Must adhere to Joint Commission National Patient Safety Goals. Work Schedule: Monday through Friday; 7:00 . to 3:30 . or 7:30 . to 4:00
