Aging and People with Disabilities (APD), part of the Oregon Department of Human Services, is seeking a dedicated and compassionate individual for a Case Aide 2 (Human Services Assistant 2).

YOUR DUTIES AT A GLANCE:

In this position, you will assist case managers in the delivery of long-term care and financial benefits to seniors and people with disabilities. This position will assist in the processing of redeterminations, reviewing financial eligibility, and assisting in the management and coordination of Home Care Workers, and may be utilized to make home visits in order to assure the appropriate continuity and effectiveness of developed care plans and service authorizations initiated by case managers. Case assistants are used as an adjunct to case managers and must utilize judgement in recognizing and assessing client's health and functional status and be knowledgeable of issues representing potential abuse and neglect.    

Major duties include:

  • Serve as primary office contact with Home Care Workers related to issues of voucher issuance, payments, and general inquiries. Triage questions or concerns for appropriate referrals to case managers. Assess HCW suitability and performance of authorized tasks.

  • Coordinate Contract RN referrals, forms, communication. Receive, distribute, and maintain facility alerts. Coordinate changes with Client Maintenance unit. Check and/or update screens on clients needing recert, including BEIN, SAVE, TPQY and others.

  • Process Interim Change reports on Mainframe, ACCESS. Work “green bar” reports. Check “odd load code” or unassigned cases and correct/assign.

  • Make home visits to obtain signatures, deliver items, perform welfare checks, etc. Assist clients in completion of necessary forms. Arrange for and assist clients on visits to potential placements. Assist client with move coordination and the actual move. Assist clients with obtaining needed household items through community resources. Assist in referrals for ancillary services, such as home delivered meals, district centers, “lifeline,” etc. Provide reminders to clients for important appointments.  Schedule interpreters.

  • Consult immediately with case manager or supervisor when identifying problematic or unsafe in-home plans or significant change in client functioning.

  • Ongoing consultation with case managers regarding client/case needs and status. Narrative entries recorded daily in electronic case file.

   

WORKING CONDITIONS

The majority of work is performed in an office environment.  However, frequent out-of-office tasks and client home visits will be required.  Must possess and maintain an Oregon Driver's license with an acceptable driving record or have reliable and efficient alternative means of transportation.  May come in contact with angry, hostile, frustrated individuals and those with behavioral and/or cognitive challenges. May come in contact with a wide variety of housekeeping standards, travel in very rural areas and may be exposed to household pets and potentially communicable health conditions.

WHAT YOU NEED TO QUALIFY/MINIMUM QUALIFICATIONS (MQ’s):

One year of experience assisting professionals who provide social program services to clients who are in one or more of the following specialized social program areas:

  • Clients who are economically disadvantaged.

  • Clients who are substance dependent.

  • Clients who are mentally or physically disabled.

  • Clients who have experienced child abuse and neglect and/or

  • Clients who are juvenile delinquent(s).

Any degree in a related mental health or human services program may substitute for six months of the required experience.

ESSENTIAL ATTRIBUTES (EA's):

  • Experience working with underserved and at-risk populations. 

  • Experience processing confidential and sensitive documents.

  • Experience working in a high production work environment completing multiple tasks with critical, competing deadlines.

  • Experience with data entry and generating reports with a database.
