  • Conduct thorough psychosocial assessments of patients with renal conditions, identifying and addressing social, emotional, and financial challenges affecting their health and treatment adherence.
  • Collaborate with the healthcare team, including nephrologists, nurses, dietitians, and other specialists, to develop and implement individualized care plans.
  • Ensure patients and families understand their treatment options and procedures.
  • Assist patients in accessing financial assistance, community resources, and support services, providing guidance on insurance issues, transportation, and housing needs related to treatment.
  • Offer individual and family counseling to help patients cope with the emotional aspects of chronic kidney disease and treatment.
  • Advocate for patients' needs and preferences within the healthcare system.
  • Educate patients and families about kidney disease, treatment options, and lifestyle changes.
  • Provide support and guidance throughout the treatment process, including preparation for kidney transplantation if applicable.
  • Maintain accurate and confidential records of patient interactions, assessments, and interventions.
  • Prepare reports and documentation as required by regulatory and organizational standards.

Requirements:

  • BLS certification from the American Heart Association
  • Covid vaccine card
  • 2 years of experience as a Social Worker
  • MD State License

Work Schedule:

  • Morning Shift
  • 08:00 AM to 04:30 PM
  • 5x8
  • 40 hours Per Week
