The Incident Management Coordinator (IMC) works under the direct supervision of the Facility Director. The IMC will supervise program planning and implementation of the Incident Management process and structure for the facility, to ensure effective decisions are made during the investigation of unusual incidents, crisis situations, staff coverage, and health/safety concerns. The IMC will identify, report, investigate, and enforce remedial actions for all unusual incidents as well as oversee and ensure quality control of the investigative process along with analysis of data related to unusual incidents and follow-up tracking of all recommendations for completion. Reviews all investigations including the written report, together with any other relevant documentation to ensure the investigation is thorough and complete and the report is accurate, coherent and meets compliance with certification requirements, regulations, policies, and procedures. The IMC will coordinate all activities related to the daily review meetings which include investigations, corrective action plans, recommendations, and minutes. Coordinates all critical incident team reviews and any required root cause analysis reviews or required corrective action plans and recommendations. Supervises and trains facility investigators, campus administrators, and other staff as needed. Works under general supervision, with limited latitude for the use of initiative and independent judgment. Some travel may be required, and work schedule may vary depending on services needed.
Essential Job Functions:
Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. May serve in an on-call status on a rotating schedule as required to meet operational needs of the facility.
Provides oversight and quality control of the unusual incident investigative process and establishes an administrative review process for discovered, non-serious injuries of unknown cause, to reduce the rates of unusual incidents at the facility. Performs investigation reviews and completes the final investigation report. Follows up on preliminary investigations and review of evidence submitted for completion of recommendations. Facilitates critical incident meetings to review high profile incidents to ensure adequate protective measures and corrective actions are implemented. Conducts unusual incident investigations in investigator's absence and as assigned by Facility Director

Serves as the liaison with Health and Human Services Provider Investigations, Office of the Inspector General, and local law enforcement entities, to facilitate compliance with statutory requirements and discuss and resolve any concerns with timeliness of investigations and false allegations. Coordinates and submits information for the Dept. of Justice settlement agreement plan of improvement and interacts and exchanges information with various state and federal agencies. Prepares reports on findings and recommendations including reports to the SSLC Director, SSLC State Office and Health and Human Services Provider Investigations.

Tracks and coordinates unusual incidents including serious client injuries, and abuse/neglect allegations, and ensures incidents are reported to HHSC Regulatory and State Office within required timeframes, as evidenced through staff training and investigation reports. Maintains database for incidents and reports, tracks recommendations and monitors all unusual incident corrective action plans to ensure corrective actions are completed.

Supervise case coordination and initial reporting with additional responsibilities that include preventive actions, determining root cause of incidents, tracking follow-up responses, and plans of correction. Assigns, prioritizes and supervises the work of the SSLC Investigators and related staff. Trains other staff, as needed, on incident reporting procedures, as evidenced through effectiveness of training and quality of product. Transmits information regarding incident management to relevant staff at the state supported living center and to peers at other centers to identify strategies the center can implement to reduce the risk of similar events occurring in the future.

Develop the facility's Unusual Incident Policy using required statutory and agency compliance standards. Evaluate all investigations to ensure compliance with policy. Revise policy, as needed, to address non-compliance trends and changes in statutory mandated compliance.

Coordinate all activities related to the daily Incident Management Review Team meeting to ensure that the meeting is operationally sound and in compliance with State Office guidelines. Documents preliminary review of unusual incidents, including documentation of team recommendations. Gathers information that will assist in the development of plans of correction for internal and external reviews, surveys and investigations.

Oversight and review of trend analysis data related to unusual incidents. Analyzes data to ensure the accuracy and integrity of the data and identifies trends and patterns that may contribute to the occurrence of unusual incidents. Produces a corrective action plan that identifies the strategies the state supported living center will implement to reduce the risk of similar events occurring in the future. Ensures implementation of the plan with the Incident Management Review Team and required staff, and monitors compliance and completion

Respond to, coordinate, and direct response to facility emergencies and works with local organizations related to disaster planning. Serves as a member of the Incident Command System and ensures Emergency Preparedness exercises are completed timely and appropriately.

Completes other duties as assigned to further the mission of the State Supported Living Center. Serves as an Executive Duty Officer on a rotating basis with other executive staff and chairs facility-wide committees as necessary.
Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.
Knowledge Skills Abilities:
Knowledge of investigative principles, techniques, and procedures; of the laws governing the activities regulated by the agency; and of court procedures, practices, and rules of evidence.
Ability to gather, assemble, correlate, and analyze facts; to devise solutions to problems; to develop and evaluate policies and procedures; to prepare reports; to communicate effectively.
Able to produce written documents using Microsoft Office programs.
Ability to accurately and clearly interpret/develop pertinent policies/procedures and technical reports.
Ability to establish and maintain effective working relationships with staff, clients, parents, members of the community-at-large, outside state and local investigation agencies or others in work context.
Ability to organize, coordinate and delegate routine and complex tasks and supervise the work of others.
Ability to develop monitoring systems, collect and analyze information, and make cost effective recommendations.
Ability to make effective decisions during unusual incidents/crises.
Ability to investigate incidents, analyze results and document recommended actions as well as ability to train other staff on investigative techniques.
Ability to demonstrate commitment to facility mission and workplace/service values.
