Provides case management and direct services for youth at the Scott County Juvenile Alternative Facility (JAF) in a trusting and therapeutic manner, performing functions required by federal and state laws and rules. Primary responsibilities include; youth assessment, development, and implementation of individual case plans, youth programming, group facilitation, transitional planning, community re-integration, and report completion. Independent professional judgement is required to ensure assessments and case plans are holistic, culturally sensitive, clinically appropriate, person-centered, strengths based, resource conscious, and achieve desired program outcomes.
Pay range shown is hiring range. Full range goes up to $103,297 with outstanding performance.
Essential Duties
Performs professional level case management services by establishing trusting, helpful, and empowering relationships with youth, families and other professionals to assist in successful stays while at the Juvenile Facility and successful transition to the community.
Independently prepares and implements individualized case plans in collaboration with youth, guardians and other professionals. Gathers and evaluates information to determine the needs of youth, collecting essential legal, social, family, psychological, and educational information relevant to each youth.
Ensures that all important information is shared with staff, schools, and partner agencies, and facilitates communication regarding youth between the referring agent and JAF.
Works in collaboration with behavioral health supervisor and youth primary to develop and implement a comprehensive treatment plan, aiming to support the youth‘s successful transition and re-integration into the community through evidence-based practices. Assesses and tracks progress toward treatment goals.
Develops and maintains professional working relationships with other agencies. Advocates for the needs of the youth, makes recommendations for placements, sets up staffing, facilitates youth‘s appointments, family visits, and therapy.
Initiates, coordinates, and facilitates programs, groups, activities, and staffing for youth and their families to include all community-based professionals working with the youth.
Co-facilitates treatment team meetings, provides input to staff in regard to strategic interventions, de-escalation techniques and ensures all staff documentation is complete. Completes treatment goals, progress reviews, and discharge summaries; maintains records; and provides documentation others as required/requested.
Utilizes appropriate crisis intervention and de-escalation techniques in response to problems and/or behaviors which place the youth, other youth, or staff in danger. Intervenes in emergency situations by enforcing rules, following procedures, responding to calls for assistance and obtaining additional assistance.
Demonstrates knowledge and understanding of human behavior, cultural diversities, and language barriers to ensure equitable service delivery.
Adheres to departmental, state, and legal standards.
Works the days and hours necessary to perform all assigned responsibilities and tasks. Must be available (especially during regular business hours or shifts) to communicate with other employees, clients, providers and any other person or organization with whom interaction is required to accomplish work and employer goals.Performs other related duties as required or assigned.
Minimum Qualifications
Requires equivalency of a Bachelor‘s Degree in Social Work, Psychology, Sociology Corrections, Criminal Justice, or related field and two years of experience in working with youth in treatment, corrections, or probation environment.
Preference for experience in working in a residential setting.
The incumbent shall provide a reliable means of transportation and a valid driver‘s license for the performance of work responsibilities. A pre-employment exam and drug test is required.
