Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Work Schedule: 8:00am to 4:30pm, Monday through Friday

Pay: Competitive salary, annual performance bonus, regular salary increases

Paid Time Off: 50-55 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year and possible 5 day paid absence for CME)

Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA

Licensure: 1 full and unrestricted license from any US State or territory

Contract: No Physician Employment Contract and no significant restriction on moonlighting

This is your opportunity for career growth and leadership development The Extended Care and Palliative Medicine Service (EC&PMS) Home Based Primary Care program (HBPC) Medical Director provides clinical leadership through direction and coordination of medical care for the patients enrolled in the program. In addition, the incumbent provides geriatric consultation or primary care in an outpatient clinic setting.

The major duties and responsibilities include, but are not limited to:

  • Collaborates with the HBPC Program Director to provide leadership to the program.
  • Coordinates comprehensive, interdisciplinary, medical care and case management in the homes of the veterans with complex medical, social and behavioral conditions.
  • Participates in the establishment of policies, procedures, and guidelines necessary for clinical care in collaboration with other members of the interdisciplinary team.
  • Oversees scheduling of advance practice providers in the HBPC program.
  • Monitors performance of medical services provided by the providers.
  • Oversees admissions to the program.
  • Provides consultation to the medical staff for patients with complex treatment or care needs in the home.
  • Collaborates in the monitoring of quality and safety concerns within the program.
  • Communicates relevant responsibilities, policies and procedures to providers
  • Responsible for maintaining medical standards of care, as mandated by accrediting bodies such as the Joint Commission.
  • Responsible for providing assessment, diagnosis and recommendations on the geriatric syndromes in the veterans he/she is consulted on.
  • The Extended Care and Palliative Medicine Service (EC&PMS) Home Based Primary Care program (HBPC) Medical Director may hold a faculty appointment at Stanford University in the division of General Internal Medicine.
