The Center for Community Health is seeking a Medical Director with a passion for the prevention of chronic diseases and the capacity to direct a novel clinical practice focused on primary prevention and multiple community programs.

Creatively and with vision, the Medical Director provides clinical, educational, research and strategic planning oversight for both clinical interventions and community health programs at the Center for Community Health and Prevention.

The Director is an M.D. specializing in health behavior change who actively seeks collaborations with other content experts. The clinical work will have an emphasis on evaluating and treating patients and worksite wellness participants to enhance their motivation to reduce the risk and improve management of chronic diseases. The community programs extend this work into the Rochester and surrounding region, providing group programming to increase awareness and education.

The Medical Director builds, trains, develops and supervises the clinical team, consisting of additional physicians, nurse practitioners, dietitians, exercise physiologists, lifestyle counselors and community health educators, on interventions for obesity, tobacco dependence, advanced cholesterol management, weight loss, diabetes prevention and lifestyle change. The Director will train staff on the integration of the tenets of Self-Determination Theory into clinical practice and community interventions, creating a unique environment for patients and participants that ultimately leads to health behavior change that prevents morbidity and mortality. In addition, the Medical Director oversees the community-based programs, collaborating with the Director of Community Health Partnerships and the Director of the Cancer Services Program.

Working at the complex intersection of clinical care and population health, the MD will be responsible for keeping abreast of current guidelines and best practices, as well as adding to generalizable knowledge regarding prevention and behavior change through research, publications, grant applications, and implementation and dissemination of new approaches to behavior change and prevention.

Primary office location is at the Center for Community Health and Prevention. Reports to the Director of the Center for Community Health and Prevention and works closely with Center Administrator. Supervises programs delivered in multiple sites throughout the region.

Specific Responsibilities:

25 % Provides complete oversight of the clinical and community health programs of the Center for Community Health and Prevention. Oversees and leads strategic planning initiatives for the development of clinical and community prevention programs. Links programs to the mission and vision of the Center for Community Health and Prevention. Addresses health disparities, bridging research activities with community health improvement programs.

Ensures all clinical activities performed by the team are compliant with standards, policies and procedures of the organization.

Identifies challenges and opportunities faced by the broader patient base and rapidly evolving health care system and implements programs to address those needs. Develops and integrates evidence-based protocols into clinical practice.

Understands how health behaviors are shaped through the integration of cutting-edge observations from population health, clinical and science-based discoveries.

Oversees implementation of community grant-funded initiatives

30% Leads research efforts using data from clinical interventions and other clinical and community data, testing new methods of implementation and dissemination, applying for grants to support new initiatives, ensuring collection of useful data, overseeing analysis and leading publication efforts. Presents at national and international conferences.

10% Leads the Center s efforts to integrate CCHP programs with UR Medicine specialty clinics and primary care offices. Ensures service coordination and continuity of care between CCHP programs and URMC departments. Grows physician referrals to CCHP programs and ensures referral process follows standard protocols.

15% Provides patient care consistent with the scope of privileges defined by the Center for Community Health and Prevention. Per policies and protocols, obtains patient history accurately using patient interviews and performs physical examinations using established physical assessment skills. Reevaluates patients at each visit. Writes medication and other medical orders. Uses patient-centered counseling approaches and lifestyle change interventions, collaborating with other health team members, as needed. Documents pertinent findings in patient record.

5% Works with Center leadership team to develop strategies and goals and explores new opportunities for growth. Stays abreast of insurer reimbursement models and trends and ensures CCHP is a first responder when new reimbursement opportunities arise.

Works with URMC leadership to educate and inform insurers of the benefits of primary prevention.

5% Identifies and leads quality assurance activities. Maintains participant/patient confidentiality and follows applicable HIPAA/RSRB requirements. Leads weekly clinical team meetings for professional development and case reviews. Serve as consultant to other members of the CCHP as part of an interdisciplinary approach to patient care and population health

5% Participates as a member of the training team that facilitates workshops on the integration of the tenets of Self-Determination Theory into clinical practice.

5% Attends relevant conferences and continuing education events to keep current on chronic disease risk reduction recommendations. Attends and contributes to weekly operations meetings and medical center strategy / planning meetings as appropriate. Contributes to the visibility of the CCHP and solidifies the CCHP as the regional go to for expertise in chronic disease prevention. Provides mentorship for clinical team, Andrus Fellow, distinction in community health students and other students and faculty who participate in educational programs and rotations through the Center.

The referenced pay range represents the University s good faith and reasonable estimate of the base compensation range for this faculty position. Individual salaries will be determined within the job s salary range and established based on (but not limited to) market data, the individual's experience, and expertise, with consideration to related position salaries.
