Position Overview:


Our client is seeking a skilled Advanced Practitioner (APP) to join their innovative healthcareteam, which is dedicated to enhancing senior care across 26 states and over 1000 senior living facilities.

This full-time position will provide direct patient care within our post-acute care settings, focusing on prevention and care coordination to optimize health outcomes for their residents.



Essential Duties and Responsibilities:

  • Deliver Providerlevel, direct patient care services, adhering to our company philosophy which aims to maintain or enhance each patient's clinical status and quality of life.
  • Perform Annual Wellness Visits and potentially Annual History and Physicals for longterm Nursing Facility residents as per facility requests.
  • Engage in clinical interventional, episodic direct care visits, and necessary reevaluation of care plans.
  • Provide continuous evaluation and treatment of chronic illnesses, ensuring medical appropriateness and efficacy.
  • Respond to changes in residents' conditions by evaluating and treating accordingly.
  • Manage medications, orders, and diagnostic tests; respond to clinical abnormalities under the guidance of collaborating physicians.
  • Facilitate and coordinate comprehensive care with other healthcare disciplines, enhancing overall medical management.
  • Maintain proactive communication with residents' families and participate in family conferences whenever possible.
  • Assume additional duties as necessary to support the healthcare team and improve patient care.

Qualifications:

  • Valid Nurse Practitioner or Physician Assistant license in Texas with current board certification.
  • Minimum educational requirement of a Bachelor's and Master's Degree in Nursing from accredited institutions.
  • Demonstrated experience in nursing, with a preference for candidates who have worked as a Nurse Practitioner or Provider.
  • Proficient in clinical and critical thinking skills, particularly applied to aged, chronically ill, and convalescent residents.
  • Strong leadership, interpersonal, and problemsolving abilities.
  • Excellent verbal and written communication skills.
  • Must be punctual, wellorganized, and able to work effectively within a team.
  • Compliance with all applicable EEOC guidance regarding vaccines and qualified exemptions is required for employment.

Compensation and Work Schedule:

  • This position operates on a unique payforperformance model based on a work Relative Value Unit (wRVU) system, offering a competitive base salary of $110,000 annually, with potential additional pay based on performance.
  • Standard operation hours are from 8:00 AM to 6:00 PM, with required oncall nights and/or weekends 34 times per month.
  • Minimum daily requirement includes 4 hours of patient care, seeing at least 10 patients.
  • Chart work and other related tasks can be completed at home.
