The Patient Care Manager supports, assists and reports to the Nurse Director. Can assume the role of Nurse Director in his/her absence.
EducationBachelor's degree in Nursing (BSN) Required andMaster's degree or Master's program enrolled Preferred Experience Two years of progressively more responsible job related experience. Required Licenses and CertificationsRN - Registered Nurse - State Licensure and/or Compact State Licensure in the District of Columbia Required BLS - Basic Life Support Required ACLS - Advanced Cardiac Life Support within 180 Days Required Certification in the clinical/specialty area must be obtained within one year of hire from a nationally recognized nursing organization within 1 Year Required Communication & Relationship Building Supports a patient first culture that fosters a high level of patient experience as demonstrated by outcomes data. Collaborates with Nurse Director to hire, evaluate promote, initiate coaching, counseling and termination of departmental personnel as needed.

In the absence of the Nurse Director, the Patient Care Manager maintains communication with the Senior Nurse Director, Associates and Medical Director or their designee for the purpose of coordinating operations between the department and the clinical division. Establishes, develops, and maintains effective relationships, working agreements and quality partnerships across the continuum to ensure quality patient care; to secure resources to support evidence based patient care; and to assure compliance with current professional policies, procedures, and protocols and nursing standards. Knowledge of Healthcare Environment/Business Skills behaviors aligned with the organization's vision, mission and values, and performance standards. Maintains continuing education files, competency assessment documentation, licensure and hospital certifications for self and associates.

Works collaboratively with the Nurse Director to seek and utilize information (including research findings) to develop, implement and manage a budget which supports organizational objectives, patient care standards and quality of care. Demonstrates fiscal responsibility with day-to-day unit productivity and unit operations. Utilizes approaches and strategies that support and optimize patient care outcomes that are based on the interdisciplinary plan of care and absolutes of IMOC and Contemporary Primary Nursing Takes action to support the National Patient Safety Goals, nursing sensitive indicators, regulatory requirements (i.e. DC Health, TJC) and other identified quality metrics.
Supports and promotes quality and patient safety initiatives. Collaborates with physicians and associates to monitor and analyzes departmental performance data and identifies and participates in hospital and/or unit-based performance improvement activities including, but not limited to, core measures, nursing quality indicators, and other patient care outcomes (e.g. Length of stay and patient experience). Delivers direct patient care as required.Leadership/Professionalism Demonstrates clinical expertise by applying evidence-based knowledge, astute clinical judgment, and decision-making that supports and optimizes patient care. Promotes an understanding of the rationale for care delivered for a defined workgroup.

Supports a professional, Patient First atmosphere and an environment of coaching and development that supports shared decision making, interdisciplinary collaboration, and a high level of patient and associate satisfaction as evidenced by outcomes data. Provides clinical leadership through role modeling professional practice behaviors, proper delegation, and management of the team by maintaining staff accountability. Evaluates the need for and effects change in the professional practice environment. Maintains and demonstrates knowledge of current trends and developments in nursing practice through a variety of activities including but not limited to reading appropriate literature, attending related seminars and conferences, and membership in a professional organization. Demonstrates actions and behaviors that facilitate the process of change necessary to support the DON strategic plan. Demonstrates commitment to Pathway to Excellence Standards through daily practice. Promotes Pathway to Excellence standards through daily practice.
