Job Summary:

Assumes the day to day coordination and implementation of the admission/readmission process. Under the direction of the Nursing Director, assesses the implementation of the initial care plan, customer service needs/satisfaction and coordinates the meet and greet resident care planning meeting.

Essential Job Functions and Responsibilities:

Receives, reviews and analyzes all admission/readmission data. Completes insurance verification and/or authorization for new admission/readmissions Enters resident demographic information into the electronic medical record Provides clinical updates to third party case managers as appropriate for continue insurance authorization Coordinate with IDT regarding required documentation for admission Act as a Liaison between the discharging facilities and Ingraham Manor to promote census stability and growth Obtains any necessary equipment and teaches any new care technique(s) to care staff, prior to admission. Reviews the admission team's room selections (i.e. reviews MDRO history, and considers roommate selection for compatibility). Performs the final room inspection for admittance approval. Audits medical records as directed by the Nurse Director. Completes special projects/assignments and other duties as assigned. Maintains confidentiality in all issues relating to and involving residents and employees. Take any steps required to ensure a safe and secure environment for residents, families and staff. Fosters educational endeavors and professional growth and development. Stays current with professional industry knowledge. Maintains ongoing and updated attendance at annual mandatory educational programs and fosters the importance of on-going education.

Maintains familiarity with all local, state, and federal regulations and standards and ensures that such standards are met. Identifies all external monitoring agencies standards' and ensures compliance as it relates to the admission/readmission process.

Organizes to conduct business.

Maintains unit structure and implements all admission policies, rules, systems, controls, and nursing standards. Makes effective use of time, space, and equipment. Demonstrates development of written goals and plans that address the admission to the facility. Provides direction in prioritizing the work flow of the admission/readmission process. Coordinates the daily admission/readmission process of the nursing unit.

Administers Human Resource functions.

Ensures a healthy employee relations environment is maintained through the actual and perceived equitable application of rules, regulations, policies, and procedures for all staff on the unit. Directs the staff that may assist in the admission/readmission process and provides feedback regarding their performance to the Clinical Nursing Director.

Maintains fiscal responsibility for the nursing unit.

Ensures thorough management of unit resources (i.e. both material and personnel), that budgetary goals are achieved while maintaining quality of resident care.

Develops and maintains an effective communications strategy at all levels of contacts.

Effectively communicates at all levels of the organization. Represents the nursing department at any assigned meetings. Serves on various committees within Ingraham Manor. Promotes resident satisfaction and staff collaboration.

Participates and implements a meaningful Quality Assurance Performance Improvement Program.

Attends and participates in the Quality Assurance Performance Improvement Program (QAPI) as requested to ensure compliance with professional and regulatory standards. Conveys a very high concern for safety and takes preventive action to minimize accidents and unsafe work practices on the unit. Continuously assesses and evaluates the quality of the admission/readmission process in order to ensure the highest possible level of care and resident/family satisfaction. Ensures that timely reports if requested, reflective of actual results, are submitted for QAPI meetings. Demonstrates "high reliability" in principals of daily practices. Qualifications

Education/Minimum Requirements:

Graduate from an accredited school of nursing. LPN or RN

State/Federal Mandated Licensure Requirements:

Unrestricted current RN/LPN license in the State of Connecticut.



Job Related Experience Requirements:



Three to five years clinical experience, some of which must be in an inpatient setting; direct management/charge experience. At least one year of supervisory experience in a long term care setting is preferred.

Ingraham Manor Mandated Education Requirements:

General orientation at time of hire. Fire/Safety/Infection Control annually. Other programs as mandated by Ingraham Manor.



Special Requirements:

Must possess good observation skills, communications skills, administrative, leadership and supervisory skills. Thorough knowledge of pertinent laws and local, state and federal regulations.

Physical Requirements:

Must be able to lift at least 50 lbs. Must be able to endure extended periods of walking, standing and sitting; and moderate amounts of twisting, reaching, bending and occasional crouching, crawling and balancing. Vision, hearing, smell and tactile sensation corrected or accommodated to normal.

Work Environment:

Regular contact with body substances and hazardous chemicals. Varied lighting, moderate noise and state-regulated temperature of 75 degrees.

Cognitive Requirements:

Ability to follow and comprehend oral and written communications; excellent interpersonal skills; ability to use judgment in problem solving.
