GENERAL DESCRIPTION. Responsible for clinic oversight, data review, chart audits, and educational tasks associated with clinic operations, and provides leadership and direction to support staff assigned to this clinic.

ADMINISTRATIVE SCOPE OF DUTIES.

Is knowledgeable of and ensures compliance with a) VHA Handbooks and Directives, b) VISN policies, c) Medical Center Memorandum and d) Service Policies and Procedures pertinent to provider operations.Is knowledgeable and capable for responding to Congressional inquiries.Is knowledgeable and in compliance with accreditation standards, including IG, Joint Commission, and other applicable survey/oversight entities.Is knowledgeable and in compliance with Safety and Environment of Care (EOC) standards and programs including the following:SDS RequirementsThe Joint Commission EOC StandardsDisposal of Hazardous WastesGEMS ProgramFacility Emergency Preparedness PlanFacility Safety PlanInfection Control GuidelinesManagement of RMEHas a working understanding and knowledge of available reports and data sources available through VistA, VSSC, DSS and other VHA data sources to facilitate management of all clinical areas within the facility.Has a working knowledge of Compass and Almanac reports.Has a working knowledge of PACT Composite Measures.Has a working knowledge of DSS reports.Has a working knowledge of applicable VSSC reports including Ambulatory Sensitive Conditions and the SAIL report.Monitors Performance metrics developed for all providers by the ACOS Ambulatory Care to drive performance and to direct desired behavior.Provides feedback to staff during recurring monthly staff meetings.Has knowledge of and acts in collaboration with Business Office leadership for non-compliance with VHA Scheduling Directives.Ensures compliance with provider profile management scheduling guidance with regards to New, Return and Flex appointment slots.Is actively engaged in facility Quality Improvement activities, including compliance with the VA credentialing/privileging process.Actively engaged in Service Recovery and has knowledge of SHEP and other patient satisfaction data sources. Uses customer service data for service improvement.Operates a sound personnel performance management system in compliance with the Union Master Agreement and HR policies including VHA labor policies. Responsible for, has knowledge of and/or ensures that the following are current:Performance Appraisal System, including performance plans, performance target goals, rating system and awards program.Time and leave policy for all staff.Grievance and arbitration procedures.Master agreement.Disciplinary action procedures/processes.AIB and other investigative options.Has knowledge of and follows conflict of interest and ethics policies related to business practices/associations, including contracting and procurement regulations.Uses MS Word or comparable word processing software to execute several office automation functions, such as storing and retrieving electronic documents and files, activating printers, inserting, and deleting text, formatting letters, reports and memoranda, and transmitting and receiving e-mail. Uses MS Excel or other comparable spreadsheet software to develop and change spreadsheets to graphically display data. Uses MS PowerPoint or other comparable graphics software to develop presentation materials. Uses the Veterans Health Information & Technology Architecture (VistA) and Computerized Patient Record System (CPRS) to access information in the Medical Center Computer System.Protects printed and electronic files containing sensitive data in accordance with the provisions of the Privacy Act of 1974 and other applicable laws Federal Regulations, VA statures and policy and VHA policy. SCOPE OF CLINICAL DUTIES. The Primary Care Physician shall be proficient in all aspects of general practice. Specifically, the provider should possess the expertise and competency to perform the following:Fully implements the processes and systems relative to the VHA Primary Care Medical Home (PCMH) healthcare system designCoordinates care for a panel of outpatients, as assigned by TVHS.Evaluates and treats acute and chronic health problems for scheduled and unscheduled patients in the clinic setting.Completes and documents history and physical exams.Provides appropriate documentation to minimally include history, physical exam findings, diagnosis, definitive care plan and disposition of the patient.Ensures that patients are appropriately closed out, including disposition that clearly delineates the next steps of care.Ensures that each patient is provided a list of current medications and an appointment list at close-out.Performs only procedures contained within their approved privileges.Prescribes medications as appropriate.Orders routine laboratory, radiological and diagnostic tests and provides initial interpretation of results.Communicates test results to patients per facility timeliness standards.Responds to questions and other inquiries from patients.Initiates or expedites requests for consultations and special tests.Provides education to the patient and family as necessary.Orders immunizations and other preventive or screening services as necessary.Screens patients to determine the need for hospitalization.Performs Basic Life Support as necessary.Communicates clinical findings to other providers using the Computerized Patient Record System.Treats patients 18 years of age or greater The full scope of practice that a specific provider may perform is defined through the medical center credentialing and privileging process.

SUPERVISORY CONTROLS. The Primary Care Physician is a licensed independent practitioner that practices based on their specific approved privileges and guidance derived from VHA, Medical Center and Service Directives/Memorandums. The Primary Care Supervisor, Community Based Outpatient Clinic (CBOC), has responsibility and clinical oversight over all clinical operations within the CBOC. The Primary Care Physician reports directly to the Chief Medical Officer for daily administration and technical oversight.

QUALIFICATIONSTitle 38 qualifications are cited in VA Handbook 5005, Part II, Appendix G.Holds the following minimum qualifications:Citizen of the United States. Non-citizens may be appointed when it is not possible to recruit qualified candidates.Graduate of a Medical College, in receipt of a MD, DO degree or its equivalent and licensed to practice medicine in one of the states or territories of the United States or the District of Columbia.Board Certification in Internal Medicine or Family Practice is preferred, but not required.Basic Life Support (BLS) Certification.Advanced Cardiac Life Support (ACLS) Certification is required if conscious sedation is performed by the provider.Must be proficient in spoken and written English.
