The incumbent serves as an entry-level case manager for the HCHV program and provides outreach and case management services to Veterans experiencing homelessness consistent with the standards identified in the latest iteration of VHA Handbook "Healthcare for Homeless Veterans (HCHV) Program".

The incumbent builds rapport with difficult to engage homeless Veterans to assist them with obtaining needed VA and community resources.

The incumbent conducts assessments to screen for psychosocial, medical and psychiatric needs; coordinates community-based services, including information and referral for additional services from other VA programs and other government or community agency programs.

The incumbent documents case activity in accordance with Joint Commission, VHA and facility policies.

Develops, documents, and implements treatment plans with Veterans to address those needs; provides outreach and limited case management to Veterans residing in Grant and Per-Diem programs and Contracted Emergency Shelter programs and assistance in providing coverage to other HCHV staff as needed.

The Incumbent establishes and maintains contact with a network of housing and material resources throughout Franklin County and surrounding areas.

Compiles information about affordability, application criteria, accessibility and eligibility criteria and advocates for Veterans to remove barriers to housing.

The incumbent is responsible for data collection, administrative record keeping, and monitoring patient or clinical indicators, acuity, and performance measurements.

The incumbent maintains current and establishes new relationships with other community-based service providers that serve homeless and formerly homeless Veterans, and when appropriate, represents VA at community service provider meetings.

Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs.

Incumbent advocates for Veterans with these agencies as needed to remove barriers to housing.
Incumbent has working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illness/injuries, psychiatric conditions and symptomatology, common medications and their effects, and medical terminology.

Incumbent has a general knowledge of Veterans' benefits and services related to special programs, service ­connected compensation and non-service-connected pension benefits.

Assists veterans with linkages to organizations that assist with the VA benefits application process.

Incumbent assists the Community Outreach Division Director with ensuring the HCHV program meets CARF, SOARS and Joint Commission requirements.

Incumbent serves a member of the CARF preparation committee and assists with associated administrative and quality management task.

Pay:
Competitive salary and regular salary increases.

When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade).


Paid Time Off:
37-50 days of annual paid time offer per year days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.

Parental Leave:


After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.


Child Care Subsidy:


After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.
