Medical Social Worker/LMSW

We are seeking a highly skilled Medical Social Worker/LMSW to join our team at PruittHealth Home Health. As a Medical Social Worker, you will play a vital role in providing medical social services to our patients and their families in their place of residence.
Job Summary:The Medical Social Worker/LMSW will be responsible for providing care to patients within the accepted standards of practice for the industry and profession. This includes completing psychosocial histories and assessments to evaluate social service needs, developing initial plans of care, and locating and obtaining resources necessary for the financial, social, medical, and emotional well-being of the patient.
Key Responsibilities:- Provide care to patients within the accepted standards of practice for the industry and profession.
- Complete psychosocial histories and assessments to evaluate social service needs.
- Develop initial plans of care in consultation with the RN Care Manager, Clinical Manager, and other members of the interdisciplinary team.
- Locate and obtain resources necessary for the financial, social, medical, and emotional well-being of the patient.
- Provide counseling to patient/caregivers to assist in coping with significant changes in the patient's condition.
- Participate in the agency Performance Improvement Program.
- Participate in staff development programs and team conferences.
- Maintain clinical records to meet federal, state, and agency regulations and guidelines.
- Masters degree in social work from an accredited school of social work.
- One (1) year of experience in medical social work.
- LMSW licensure.
- Valid drivers license and automobile liability insurance.
- Current CPR certification.
