Summary

The Health Care for Homeless Veterans and the Community Resource and Referral Center incumbent within the San Francisco VA Health Care System (SFVAHCS) identifies and engages Veterans who are experiencing homelessness, marginally housed and/or at risk of homelessness, and/or who underutilize VA services. The incumbent provides Veterans with a range of social work services, including assessment and referrals; clinical treatment; short-term clinical case management; advocacy; service coordination.



Duties

This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until filled or September 30, 2024. The initial cut-off date for referral of eligible applications is 15 days from announcement open date. Eligible applications received after that date will be accepted on an ongoing basis and qualified candidates will be considered as vacancies become available.

VA Careers - Social Work:

Major Duties Include but are not limited to the following:

  • Screening/Assessment: Demonstrates the ability to independently conduct a psychosocial assessment with a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds; Develops assessments in collaboration with the interdisciplinary treatment team, the Veteran, available family/friends/caregivers and collateral sources such as CPRS, HOMES, and agencies with appropriate ROI's; Identifies family/caregiver stressors and provides specific interventions.
  • Advance Directives: Provides education on advance directives and advanced care planning.
  • Treatment Planning/Goal Setting: Formulates treatment plans and implements plans, identifying problems, strengths, weaknesses, coping skills and assistance needed; Possesses knowledge of medical and mental health diagnoses, disabilities and treatment procedures to formulate treatment plans and provide therapeutic interventions.
  • Referrals/Linkage Services: Maintains a current network of internal and external resources to educate the family and/or family members/caregivers and assist with appropriate referrals; Makes appropriate referrals to community and other agencies and coordinating services as needed; Acts as an advocate with appropriate VA and community service providers and agencies when it serves the best interest of the Veteran and family members/caregiver.
  • Counseling/Therapeutic Interventions: Independently implements different treatment modalities when working with individuals, families, and groups to achieve treatment goals; Provides Veterans with ongoing supportive counseling.
  • Case Management/Care Coordination: Provides case management and care coordination to facilitate appropriate delivery of health care services, incorporates complex multiple causation in differential diagnosis and treatment of Veterans, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice.
  • Consultation/Training: Provides consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
  • Administrative Responsibilities: Serves on committees, work groups, and task forces at the facility and VISN level or in the community; Documents all Veteran contacts in the electronic record using appropriate formats and templates; Participate in various program and Social Work Service administrative and clinical meetings.
Work Schedule: Monday - Friday, 8:00am - 4:30pm (Tour of Duty to be determined upon hire)

Telework: Available - Partial telework possible, dependent upon program needs.
