Summary

The Social Worker Homeless Patient-Aligned Care Team (HPACT) Program Coordinator is located at the San Francisco VA Health Care System's Outpatient Clinic in Oakland. The incumbent works with members of the multidisciplinary HPACT team, providing a range of social work services.



Duties

This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until filled or September 30, 2024. The initial cut-off date for referral of eligible applications is 15 days from announcement open date. Eligible applications received after that date will be accepted on an ongoing basis and qualified candidates will be considered as vacancies become available.

VA Careers - Social Work:

Major duties include but are not limited to the following:

  • Program Coordination: Demonstrates Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring; Knowledge and ability to write policies, procedures, and/or practice guidelines for the program; Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program areas; Ability to provide training, orientation, and guidance within clinical practice; Identify areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluation.
  • Screening/Assessment: Develops a bio/psycho/social assessment of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, available family members and significant others.
  • Treatment Planning/Goal Setting: Completes an initial assessment and Individualized Service Plan (ISP).
  • Referral to Service Providers: The incumbent is the subject matter expert on VA and/or community resources, and collaborates with other service providers in assessing the Veteran's needs.
  • Advocacy: Understands the intimidation of bureaucracy and will act as an advocate when it serves the best interest of the homeless Veteran.
  • Community Planning: Coordinates community planning with the homeless Veteran.
  • Education/Health Promotion and Prevention: Provides education related to VA and community resources, entitlements, advance directives/living wills and will refer Veterans to the appropriate interdisciplinary team member for identified health education needs.
  • Supportive Counseling: The incumbent provides the Veterans with ongoing supportive counseling.
  • Administrative Responsibilities: Enters all Veteran contacts in the electronic records

    system using appropriate formats and templates.
Work Schedule: Full-time; Monday - Friday, Tour of Duty to be determined upon hire.

Telework: Available - Partial telework possible dependent upon program needs.
