Responsibilities


The Brook Hospital Dupont is currently looking for a PRN Weekend Social Worker / Therapist to add to our team The hours for this position are Saturday and Sunday 8a-4p.

This position requires at least one weekend per month.

This position pays $30 - $35 per hour.

The Brook – Dupont, located in Louisville, KY, is an 88-bed hospital offering a full continuum of inpatient, extended care, partial hospitalization and outpatient services for children and adults.

The PRN Social Worker / Therapist is a member of the multidisciplinary treatment team

Major job functions include:

obtaining psycho-social assessments; conducting family/group therapy on assigned cases; complete documentation per hospital policy and procedure; case coordination; coordinate discharge planning; assist in developing and maintaining community resource information; performs marketing functions as assigned; assists in the maintenance of the program environment in compliance with policies of safety and security; ensures good clinical judgment to ensure optimum patient care; participate in continuing education activities.

Job Duties

  • To provide family and group therapy on assigned cases, including obtaining psychosocial assessment information, referral source contact, case coordination and discharge planning
  • Ensures that a preliminary discharge plan is formulated with the treatment team and family, as an integral part of treatment
  • Ensures that therapies conducted are consistent with the goals and philosophies of the program, and supportive of each patient's individual treatment plan
  • Documents discharge planning one (1) time weekly, at a minimum Completes and signs the Discharge Contract
  • Assures there is a clinical therapy note in chart one (1) time weekly, if not conducting family therapy Completes group notes in a timely manner
  • Completes all treatment plans in a timely manner, and includes all required clinical services objectives Serves as Case Manager for assigned cases
  • Completes clinical service requirements on Treatment planning forms

Qualifications

Required:
Master's Degree from an accredited school in the Behavioral Sciences

Preferred:
licensed or under supervision in state of Kentucky (CSW/LCSW CADC/LCADC, MFTA, LMFT, LPCA, LPCC, LPA, LPP)

Preferred:
One year inpatient behavioral health work experience.
