JOB REQUIREMENTS:
  • Current RN license and good standing with the Delaware State Board of Nursing.
  • Certification in Case Management (ACM or CCM) is a plus
  • Minimum of 1 year full time experience in Care Management within the last year.
  • Proficient in supporting patients' access to post acute care services and appropriate utilization of resources. Collaboration with post-acute care providers to achieve optimal health and ensure patient needs are met in the community.
  • Must show competency with assessment, discharge planning/intervention, implementation, care coordination, monitoring and evaluation of the plan of care.
  • Proficient in managing a caseload of high risk, complex needs and/or catastrophic patient(s).
