The Community Health Workers play a crucial role in providing social support and advocacy to high-risk low-income patients to help them achieve their health goals.
Responsibilities
- Initiate contact with eligible patients telephonically and/or in person and conduct social needs screenings.
- Provide assistance to patients with their social and health-related needs through telephonic, in-home, or community-based visits.
- Advocate for patients, connect them to community resources, and collaborate with care teams to address their needs.
- Assist patients with applications for community-based programs and ensure they are connected to their medical care team.
- Attend meetings, prepare reports, and adhere to organizational standards of behavior.
Qualifications
- Bachelor's degree in a health-related field or 3+ years of experience in health/hospitality field.
- Proficiency in MS Word/Google Suite and computer data entry.
- Excellent communication skills and ability to carry out instructions.
- Comfortable with telephonic outreach, home visits, and community outreach.
Preferred Qualifications
- Prior experience as an outreach worker or related role.
- Strong time management skills and bilingual/multilingual abilities.
Licenses and Certifications Required
- Valid driver's license and reliable transportation.
