Job Summary We are searching for a Staff Nurse
  • Outpatient someone who works well in a fast paced setting.
In this position, you will provide quality nursing care to patients by assessing, planning, implementing, coordinating, educating, and evaluating age-appropriate care in a manner that incorporates family centered care and supporting end-of-life situations.

Austin, TX 78731 Responsibilities Uses appropriate evidence-based assessment, complex interviewing techniques and instruments related to ambulatory care nursing in collecting pertinent patient, family, or population specific data.

Collects subjective and objective health status data from multiple sources (patients, caregivers, members of the health care team, documents records, and other relevant sources).

Prioritizes data collection activities based on the patient's, groups, or population's immediate health need or the nurse's judgment of anticipated patient needs.

Synthesizes available data, information, and nursing knowledge relevant to the presenting health situation to identify patterns and variances.

Prioritizes the data and information collected based on the patient's or population's condition and preferences, the situation, and/or anticipated needs.

Documents the information and data collected in a retrievable, understandable, and readable format.

Develops and continuously updates an individualized plan for patients seeking care for health promotion, health maintenance, or health-related situational problems.

Includes the patient, family, appropriate caregivers, and other health care professionals in making shared decisions about prioritizing plans and strategies.

Considers patient or population needs in terms of age, gender, race, cultural values and practices, ethical and legal considerations, economic impact, and environmental factors, as well as the anticipated risks and benefits of interventions for individualized plan development.

Documents the plan using standardized, understandable, readable language and recognizable terminology while using the plan to provide direction for other clinical members of the ambulatory nursing health care team.

Prioritizes interventions and implements the plan of care in partnership with the patient, family, caregivers, and other health care professionals in a timely manner.

Utilizes evidence-based nursing interventions during clinical visits, telephone encounters, and electronic communications according to state regulations, regulatory agency standards, and organizational policies and procedures.

Uses available organizational technology and document all nursing care and services implemented in the patient health record. Coordination of Care.

Organizes components of the plan and delegate appropriate interventions to care team members based on their demonstrated competency, state practice acts, available evidence-based guidelines, regulatory and governmental standards, and organizational policies with defined outcome measures.

Conducts a reevaluation and follow-up of care delivered based on the patient's needs, the patient's condition, or the patient's request.

Provides relevant information across levels of care within one or more health care systems and when the patient's care is transferred between and among different specialties and/or within one or more organizations.

Facilitates continuity of care using the nursing process, multidisciplinary collaboration, and coordination of all appropriate health care services and community resources across the care continuum.

Manages the patient's care in order to maximize independence, quality of life, options for alternative care and transitions of care.

Documents all coordination of care in the health care record. Educate patients, families and community. Orients the patient, family, caregiver to the health care delivery system, services, access to care, and available resources.

Supports patients, family and caregivers in developing skills for self-efficacy to promote, maintain, or restore health, such as healthy lifestyle tips, risk-reducing behaviors, age and developmental needs, daily living activities, and preventive care.

Uses health teaching methods and approaches appropriate to the situation that recognize the age, gender, race, ethnicity, diversity, and cultural norms of the individual patient.

Incorporates the patient's, families', and caregiver's ability to understand and participate in the plan of care. Documents all health teaching and promotion in the health care record.

Conducts a systematic, ongoing, and criterion-based evaluation of the outcomes of nursing care in relation to the structures and processes prescribed by the plan and the indicated timeline.

Integrates current care nursing-based evidence into the evaluation process. Includes the patient, family, caregiver, and others involved in the care situation in the evaluative process.

Evaluates, in collaboration with the patient/family/caregiver, the effectiveness of the planned strategies in relation to patient responses and the attainment of the expected outcomes.

Uses ongoing reassessment data to revise the diagnoses, expected outcomes, and implementation of the plan as needed.

Documents results of evaluation and reassessment of patient status, evidence of patient and family participation, patient responses, and the rationale for any revisions in the plan of care.

Disseminates the results of the evaluation to the patient, caregivers, and others involved in the care or situation, as appropriate, in accordance with state and federal laws, regulations, and organizational policies.

Contributes to Positive Patient Experience.

Demonstrates positive and effective interpersonal skills when dealing with patients, families, visitors, peers and other healthcare team members to generate a feeling of trust and caring.

Maintains personal professional records that include evidence of current licensure, evaluation and validation of clinical competence, continuing education, and any certification or verification of special skill sets.

Adheres to standards and applicable codes of conduct that govern behavior among peers and colleagues to create a work environment that promotes cooperation, respect, and trust.

Acts as a resource to team members; participates in the development of others. Oversees the care given by others while retaining accountability for the quality of care given to the patient. Works at alternative locations as needed to support patient care at the discretion of direct supervisor/manager or designee. Performs other duties as assigned.

Qualifications Being fully vaccinated against COVID-19, including any booster dose(s) of the COVID-19 vaccine recommended by the Centers for Disease Control when eligible, is required for all employees at Texas Children's unless approved for a medical or religious exemption.

Required Bachelor's Degree in nursing, associate degree in nursing with acceptance or current enrollment in a BSN program and signed agreement or an associate degree in a related field accepted by the Texas Board of Nursing for the purposes of obtaining and maintaining an RN license.

Technical diploma in nursing with acceptance or current enrollment in a BSN program and signed agreement . Other Must be currently enrolled in a nursing bridge program working toward an MSN. Verified BSN-equivalency or a signed agreement is required. 1-year Professional nursing experience Required Licenses/Certifications RN
  • Lic-Registered Nurses by the Texas Board of Nursing or Nursing Licensure Compact BLS
