join us at our new integrated family care clinic, Health Solutions Family
Medicine at Perry Hall, located at 1591 Taos Rd. in Pueblo, CO.
What You'll Be Doing
As a Clinical Care Specialist, you would be a vital contributor to the
health and wellbeing of children, youth, families, and adults in Pueblo.
As a Clinical Care Specialist, you would also work towards building and
maintaining effective relationships with community partners.
Specifically, the Clinical Care Specialist
Identifying stressors and precursors leading to the current crisis
Providing education regarding mental health diagnosis
Providing consistent support and availability until the
child/adolescent/family/individual have achieved their goals.
Offering children, youth, families and adults, community resources
including peer support activities.
Reminding youth and families of their appointments with medical doctors
Providing and arranging transportation for scheduled appointments
Maintaining accurate, complete, and up-to-date records according to Health
Solutions standards
Maintaining competency and proficiency with the electronic health record
Performing other job-related duties assigned by the director or his/her
designee.
Will be required to cross train in other duties as allowable by licensure.
Physical requirements
Requires standing, sitting, driving, and walking for prolonged periods
of time.
Occasionally lifts and carries items up to 20 pounds.
Frequently exposed to stressful situations.
Frequently operates a computer and other office equipment to record health
data.
Constantly communicates with clients and others.
Occasional exposure to communicable diseases and bodily fluids
Occasionally drives between Health Solutions facilities or for other assigned
duties.
Requires occasional driving in adverse weather conditions.

What We're Looking For The Must-Haves
Baccalaureate degree in social work, psychology, education, or closely
related field.
Experience working successfully with a multi-disciplinary team, especially
in an integrated primary care setting.
Ability to work in a fast-paced primary integrated healthcare practice.
Experience providing clinical services to patients of all ages.
Experience and comfort working with a wide range of behavioral health
conditions and presentations to include substance use disorders.
Experience working with severe and persistently mentally ill adults, and
experience with various behavioral health diagnoses including substance use.
Good communication skills, both verbal and written.
Exceptional time management skills.
Flexibility and ability to easily adapt to change.
Well-developed documentation skills.
Computer proficiency.
Valid Colorado driver's license, and access to reliable transportation
during work hours.
