Summary

The HBPC Nurse function is to provide quality nursing care and expected to comply with all mandated annual review requirements, rotate to other clinical areas as assigned and take responsibility for attending in-services and continuing education activities.

The nurse will travel in the community to provide care to patients in the home setting.

The initial appointment is but is not limited to HBPC and future assignments may be at the discretion of the Program Director/Nurse Manager.

Duties

Duties of this position include but are not limited to: The Registered Nurse at this level meets the basic requirements for entry level nurses.

The RN will carry tasks out in the Veteran's home to include performing case management; patient assessments; participate in interdisciplinary team process; performing skilled nursing tasks (i.e., blood draws, injections, dressing changes and wound care); completes and submits clinical notes; observes patient for changes in conditions and reports appropriately; provides education instructions to patients and their designated significant other; performs other professional nursing duties as assigned and within their scope of practice.

This position requires that the employee operate a government vehicle.

Responsible for ensuring his/her actions contribute to a mode of continued accreditation readiness regarding Joint Commission (TJC), Office of Inspector General (OIG), Commission on Accreditation of Rehabilitation Facilities (CARF), VA surveyors, Laboratory College of American Pathologists (CAP), and other survey teams.

Can articulate standards of care and practice, policies, and procedures/SOPs; documentation is complete and timely; has situational awareness of safety and other environment of care issues; initiates corrective actions; maintains safety and infection control standards; strictly protects verbal, written, and electronic patient personal and health information; maintains a professional, healthcare business environment; participates in and can articulate performance

improvement data and how it impacts patient care.

Work Schedule: 8:00am to 4:30pm (Monday through Friday)
