The work of the position supports the Cancer Committee in development and sustainment of the CoC standards and cancer-related quality improvement initiatives.
The incumbent leads the required annual QI studies. The designated person would be responsible for ensuring allrequired documentation is in the committee meeting minutes and meets the CoC requirements. The incumbent manages and reports cancer data and supports cancer related program development.
The incumbent serves as a valuable resource for integrating cancer-related services with the ultimate goal of improving cancer care at the VANEOHS.
Has overall responsibility for coordinating all Cancer Committee activities to ensure full compliance with the American College of Surgeons Commission on Cancer standards.
Responsibilities include but are not limited to:
Survivorship coordinator duties in Hematology/Oncology.
Works collaboratively with the Cancer Committee Chair, Cancer Liaison Physician, required CoC Coordinators, and all members of the Cancer Committee on the various cancer program activities.
Ensures that hospital leadership is apprised of key cancer program activities. Bi-annual reporting of Cancer Committee to the Medical Executive Committee.Assists with the preparation and documentation of all Cancer Committee meeting activities (agenda, minutes, attendance, quorum, reports, etc.). Ensures that membership is established, and cancer committee coordinators are named at the beginning of each calendar year.
Ensures that all reports presented contain all required elements to meet the intended standard.
Assists in tracking the Cancer Committee annual goals and ensures that target timelines and measures are being met.
Works collaboratively with the Pathologists to ensure that all required College of American Pathologists (CAP) elements are documented for surgical pathology resections using the synoptic format.
Assists with the preparation and presentation of the CAP compliance report.Works with the Pathologists and Surgeons to ensure compliance with the Operative Standards.
Assists the Oncology Nursing Leadership from Inpatient and Outpatient Nursing in the preparation and presentation of the report on oncology nursing education and oncology nursing competency completion.
Works as a Survivorship Program Coordinator to identify the Survivorship Program team members, choose three programs that will be offered to cancer survivors in a given year, document the number of cancer survivors that were referred to the three identified programs, report the findings annually to the Cancer Committee.
Works with the Cancer Conference Coordinator to prepare the annual Cancer Conference Coordinator Report which includes number of cancer conferences held, cancer conference physician specialty attendance, number of cases presented, number of cases that includeda discussion of cancer stage and evidenced based treatment guidelines, clinical trials, genetic testing, and supportive care services.
Works collaboratively with the Psychosocial Coordinators to manage the Psychosocial Distress Screening Program and to complete the annual report of all cancer patients that were screening for psychosocial distress annually.
Assists with the preparation and presentation of annual reports. Works collaboratively with the Cancer Liaison Physician on various reports.Works with the Community Outreach Coordinator with the preparation and presentation of the summary on community outreach activities. Ensures that the required cancer prevention and cancer screening activities are planned and completed annually.
Works collaboratively with the Cancer Liaison Physician, the Quality Improvement Coordinator, and the cancer committee to plan the annual quality improvement studies and ensure that they are initiated, completed, and reported.
Is involved in planning the quality improvement(s) based on the study results.Oversees the cancer registry systems by working collaboratively with the Cancer Registry Contract Service
Oversees all aspects of the triannual ACoS CoC Site visit.
Works collaboratively across service lines to improve cancer care at the facility.
