Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.



Duties

This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until September 30,2024. The initial cut-off date for referral of eligible applications will be 10 days after the JOA is posted, with subsequent referrals will be contingent on the availability of vacant positions. Eligible applications received after that date will be referred at regular intervals OR as additional vacancies occur on an as-needed basis until positions are filled.

The Registered Nurse will function in the Community Living Center (CLC) section of the Geriatrics, Extended Care and Rehabilitation Service. The RN (CLC) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs. Primary duties include but are no limited to:

  • Documents assessment and interventions by policy and practice standards (CPRS)
  • Conducts bedside, patient centered rounds utilizing SBAR tool and all transition of care
  • Mentors and precepts new staff and students.
  • Crisis intervention for unique population
  • Manages patients requiring hospice and palliative care services
  • Demonstrates sound techniques in complex nursing care procedures and basic care of the - Residents in a Long-Term care setting.
  • Demonstrates knowledge and skills in ostomy and wound care management
  • Provides care to residents requiring palliative, short tern, skilled care, respite, and restorative care.
  • Provides care using the HATCH model; puts the needs, interests and choices of resident's patients and clients at the center of care.
  • Operates specialized therapeutic and life support equipment
  • Collaborates with members of the interdisciplinary team, ancillary services residents and family when planning and implementing the patient's plan of care.
Preferred Experience: One year experience in Long Term Care or Med Surg.
