  • Accurately recognizing, reporting, and recording relevant patient information

  • Appropriately performing, reporting, and documenting assigned nursing care in the Primary Care Clinic
  • Fosters an environment of respect for staff, Veterans, and visitors
  • Responsible for patient and clinic flow: organizing and attending huddle, patient check in, patient notification and communication of any clinic delays, and patient check out if necessary
  • Adheres to standards of practice and safe technique in preparation and administration of medication
  • Reviews daily care schedule for patient care needs and previsit planning
  • Promptly recognizes urgent or emergent patient care situations, reports and documents findings, and effectively intervenes as directed when appropriate
  • Aid in patient education based on the patients goals, provides patient and family health education to include selfmanagement, prevention, and wellness
  • Invites contact and is easy to approach
  • Maintains current clinical competencies
  • Cultivates a positive work environment
  • Abide by MCP's, policies, standard processes, SOW's, and SOP's
  • Conducts daily glucometer quality control testing every morning as assigned
  • Completes secure messaging through MyHealtheVet (within 24 hours/1 business day)
  • Utilizes message manager to return phone calls within 4 business hours, documents in CPRS and marks call complete.
  • Hospital discharge and twoday discharge (both VA and VACC) followup calls completed within the designated timeframe with appropriate documentation.
  • Closes all encounters or documents patient noshow in EHR by close of business on date of appointment
  • Clinical scrub of all clinic schedules completed daily and weekly
  • Positively participates and presents pertinent information in both PACT/Nursing team huddles
  • Retrieves clinic appointment list and prints as needed
  • Completes documentation and signs notes in EHR by close of business (unless approval of delayed documentation is received by leadership)
  • Performs preprocedure timeout correctly and ensures proper documentation including chart review for signed consent
  • Completes all applicable clinical reminders at each visit
  • It is expected that communication sent electronically (i.e. email, teams, etc.) is checked and responded to throughout the day; action completed on all items by COB
  • Adheres to PACT nursing standing delegation orders and PACT roles and responsibilities
  • Performs phlebotomy and processes specimens appropriately
  • Follows scheduling directive and completes scheduling duties as instructed
  • Orders supplies and medications as directed by managing services
  • Ensures clinic remains compliant with Joint Commission standards

Work Schedule:
Monday to Friday 8:00am to 4:30pm

Pay:

Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade).

Paid Time Off: 37-50 days of annual paid time offer per year days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.

Parental Leave:

After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.


Child Care Subsidy:
After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $
