Job Title: Licensed Practical Nurse

Avamere Communities is seeking a skilled Licensed Practical Nurse (LPN) to join our team. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our healthcare facility.

Responsibilities:
  • Complete accurate and timely record keeping for patient admissions, transfers, and discharges.
  • Coordinate with nursing staff and support personnel to plan daily services, programs, and activities that meet patient needs.
  • Administer medications and review medication cards for completeness and accuracy.
  • Review patient care plans and make revisions as needed to ensure quality care.
  • Provide leadership and guidance to nursing personnel and make daily rounds to ensure staff is providing direct care in accordance with patient care plans.
  • Develop and maintain positive relationships with patients, families, and healthcare team members.
Qualifications:
  • Current, unencumbered LPN license in the state of Oregon.
  • Nursing degree from an accredited college or university.
  • Knowledge of nursing and medical practices, laws, and regulations.
  • Experience with Electronic Medical Records and computer documentation systems.
  • Ability to maintain confidentiality and adhere to HIPAA guidelines.
