Summary

The HUD-VASH Case Manager (RN) collaborates closely with provider(s), healthcare professionals, VA and non-VA health care providers and organizations. Provides ongoing professional assessment, coordination, planning, and advocacy on behalf of the Veteran to ensure that necessary clinical services are received in a timely and cost-effective manner.



Duties

The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran. This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being.

The HUD-VASH Case Manager duties include, but are not limited to, the following:

  • Responsible for providing case management for a caseload of veterans in the HUD-VASH program.
  • Responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements.
  • Actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences.
  • Performs site visits of HUD-VASH apartments to ensure that veterans reside in safe environments in compliance with local housing codes.
  • Verifies the veteran's status, eligibility for VA medical care, and eligibility of program participation.
  • Responsible for ensuring that homeless veterans are considered as primary participants for program acceptance.
  • Responsible for collecting and submitting HUD-VASH Program participant data as outlined by NEPEC evaluation procedures.
  • Responsible for providing HUD-VASH clinical case management.
  • Conduct Mental Health RN assessments to determine needs of veterans and family members.
